So, we’ve committed to something: When you work with us, we’ll avoid guilt trips and finger pointing, and our discussions will never center around what you should have done. Instead, we’ll turn every ounce of our focus towards what YOU CAN DO.
You’ve found your way to the Healthy Lifestyle Center, and that means you’ve already taken a huge step to improve your health. We want to do all we can to help you keep moving forward.
You’re counting on us to help you. Here’s what you can expect:
1. We take the time to know you and understand your goals.
When you visit us at the Healthy Lifestyle Center (HLC), we’ll begin by asking you to fill out an intake assessment so we can get a basic understanding of your health and lifestyle habits. Everything you share remains confidential.

2. We don’t give generalized advice or trendy action steps.
After we understand your healthcare needs and goals, we’ll introduce you to services in our community that are already addressing your specific health needs.
We’ll also use your initial assessment to build a team of experts who will work one-on-one with you at the HLC. These experts will give you all the personalized time and attention you need as you set goals and reach them!
3. We’re your partners for as long as you need us.
Each visit will be targeted to your interests, your lifestyle behaviors, and your healthcare needs. If you have a medical care provider, we’ll work closely with them to ensure that our plan of action fits with your existing medical plan. If you don’t have a medical care provider, we’ll rely on our team to empower you with safe and healthy lifestyle changes.
